Registering with a family doctor

Once you have found a family doctor or have been assigned one, you must register with that doctor. You must fill out the registration form provided to you by your doctor. Registration with a doctor is voluntary and free. 

 

Cancelling a registration with your family doctor

Registration with a family doctor may end in some cases:

  • If you register with another doctor.
     
  • If your doctor is no longer available to follow you. For example, if they leave the practice or retires. To request another doctor, contact your  area waiting list. You will retain your current family doctor until you have a new one.*
     
  • If you move and can no longer get to your current clinic. Contact your  area waiting list in your territory to request another doctor. You will retain your current family doctor until you have a new one.

You must cancel your registration with your current doctor. In doing so, you may lose access to services offered by your usual clinic. To cancel your registration, contact  Régie de l'assurance maladie du Québec (RAMQ). You may then register on the waiting list through the  QFDF or by telephone by contacting your  area waiting list.

*In some situations, you may not be able to request a new doctor while retaining your current one.
