Monday to Friday : 8:30 a.m. to 4:00 p.m.

Nutritionist, Internist, Cardiologist, Endocrinologist, Neurologist and Nurses
Program:
Bilingual service
- Post- hospitalisation ACS: follow up with patient on the phone (7 days)
- Assessment over the phone by nurse: management of patient symptoms, Wounds Medication, education (If needed)
If patient feels unwell refers to ER.
Goals:
Prevention of cardiovascular diseases (Hypertension, Chest pain, Dyslipidemia, Metabolic Syndrome, Diabetes).
Eligibility criteria for patients:
Only admitted through referral by physician.
