Training form and procedure
Our establishment recognizes the importance of maintaining and developing the skills of its staff in accordance with organizational mandates and priorities, thereby improving the quality, effectiveness and efficiency of care and services provided to the population.
Individual training
We would like to remind you that it is essential for every staff member participating in training on an individual basis—with the exception of training completed through 
ENA—to report their participation using the form below. It is the individual’s responsibility to report their training.

Individual training request (This form is optional—you may also obtain your manager’s approval via email.)
Group training
For any group training sessions, please download and complete the attendance sheet below and attach it to the Training participation confirmation above.
