COVID-19 vaccination

Eligibility

  • Pfizer vaccine: must be between the ages of 5 and 29 to receive the Pfizer vaccine.
  • Moderna vaccine: offered to people 30 years and over.

People 18 years and older are now eligible to have a fourth dose of the COVID-19 vaccine. People must wait a minimum of three months between their third and fourth dose. In addition, three months should elapse between being diagnosed with COVID-19 and the next dose. 

To receive the third dose of the COVID-19 vaccine (booster), you must have received your second dose of the vaccine at least 3 months ago. 
In the context of the current epidemiological situation, it is now recommended that everyone who wishes to get a third dose of the vaccine against COVID-19 be able to do so as soon as possible, including those who were recently infected with the virus. The booster dose offers a better protection against the Omicron variant. Those who had COVID-19 recently can get their third dose once their illness is over, i.e. when their symptoms have finished.

Eligibility criteria for the booster shot will be strictly enforced at our vaccination clinics.

An appointment can be made online for the vaccination sites at Décarie Square, Parc-Extension (7101 av. du Parc) and the Goldman Herzl Family Practice Center at  quebec.ca/covidvaccine or by phone at 514-644-4545.

How can an appointment be made for a vaccine?

Appointments can be made online through  quebec.ca/covidvaccine or by telephone at 514-644-4545 between 8:00 a.m. and 6:00 p.m., Monday through Friday. To meet public health requirements and ensure social distancing, please arrive 10 minutes prior to your vaccination appointment. This will avoid unnecessary line-ups and help ensure vaccinations are administered efficiently and effectively.

A healthcare card is not required for vaccination, but if you have one please bring it with you. Vaccination is free.

Vaccination sites

Décarie Square (6900 Décarie Boulevard)

  • Open from Monday to Saturday from 10:00 a.m. to 6:00 p.m. with appointment, and from 10:00 a.m. to 5:30 p.m. without appointment; closed on Sundays
  • Vaccination is available via walk-in or with an appointment for those aged 18 and over.
  • Vaccination is available with an appointment for those aged 5 to 17 years old.
  • Pfizer vaccine for those aged 5-29
  • Moderna vaccine for those aged 30+
  • The site will be open for vaccination registration and obtaining QR codes from Monday to Saturday from 10:00 a.m. to 6:00 p.m. 

Parc-Extension vaccination site (7101 Parc Avenue)

  • Open from Monday to Saturday from 10:00 a.m. to 6:00 p.m. with appointment, and from 10:00 a.m. to 5:30 p.m. without appointment; closed on Sundays
  • Vaccination is available via walk-in or with an appointment for those aged 18 and over.
  • Vaccination is available with an appointment for those aged 5 to 17 years old.
  • Pfizer vaccine for those aged 5-29
  • Moderna vaccine for those aged 30+

Goldman Herzl Family Practice Centre vaccination site (5790 Côte-des-Neiges Road)

  • Monday to Friday from 8:30 a.m. to 4:30 p.m.
  • Pfizer – first, second and third dose, available with an appointment for those 12 and up

Do the residents of our CIUSSS have to make an appointment at the vaccination site closest to their home?

Residents are free to make an appointment at whatever site they prefer.

Is it obligatory to have a health card to be vaccinated?

All residents are eligible to be vaccinated. If you have a health card, please bring it with you. If you do not, please bring identification demonstrating that you meet the specific criteria to be vaccinated.
