Register online to add your name to the waiting list for a family doctor.
A printed registration form is also available at the reception of CLSCs René-Cassin, de Benny Farm, Métro, de Côte-des-Neiges and de Parc-Extension.
Updating a registration in the Quebec Family Doctor Finder (QFDF) or area waiting-list
If you need to update your registration or change your contact information, changes can be made directly on the QFDF. Your registration will be updated and will not affect your place on the waiting list.
To update your address, you must contact the Régie de l'assurance maladie du Québec (RAMQ) at 514-864-3411 or make the change online through the Service québécois de changement d'adresse (SQCA).
If you want to make a change regarding your health condition, you can also report it directly to the QFDF. You will also be able to ask for an assessment of your health status by a nurse, if this service is offered by your area waiting list. The nurse will be able to update the priority assigned to your registration according to your health condition.
